At no cost to you or the union, Teamsters Local Union 638 has continued the $2000 Accidental Death and Dismemberment Benefit for each member. In addition, you are eligible to participate in the Health Services Discount Plan.

This plan allows discounts with providers in eye care, chiropractic care, prescription drugs and hearing aids. Other options include: dental, vitamins, travel and assist, and a 24-hour nurse line are also available. The benefits are being provided through the cooperation of Local 638 and American Income Life.

Notification of these benefits is mailed by the union to our members. All members interested in the optional coverage should return the reply card. As always, there is no obligation and the $2000 AD & D Benefit is provided whether you return the reply card or not.
